Incident Form

The following form is to report an incident that violates our Community Standards Policy. We take all disclosures seriously and will respond as quickly as we are able. Our first step with any report will be to prepare an individualized plan to address the situation, and then follow up with the person who received harm to make them aware of our plan and confirm whether or not the plan is acceptable to them.

The following questions are built to help us respond appropriately to address an incident. The more information we have, the better we can respond, but please know that you are welcome to share or ignore as much as you would like.

For those interested in initiating a resolution process, what information would you like to be shared with the person(s) involved? (Please note; for a person to take responsibility for their harmful actions in a meaningful way it is best if they are communicated to with their specific actions, and how they harmed someone.) *

A specific description of the incident in which they caused harm (including who was harmed)

A general description of their actions which caused harm, so as to not identify the person who they harmed

How their actions made you feel

What you would hope to see as a resolution

Please keep the disclosure completely private from this person. Only communicate that some of their actions have been flagged as inappropriate.
